If you have not previously filed a corresponding disciplinary complaint, a completed, signed disciplinary complaint form must accompany your Statement of Claim. The disciplinary complaint form is a requirement even if the attorney named on the claim form has been suspended or disbarred. If you are alleging that your former attorney misappropriated your money or property, the Office of Disciplinary Counsel must be aware of the allegation in the event the former attorney would ever seek reinstatement.
The only time a corresponding disciplinary complaint is not required is if the attorney named on the Statement of Claim is deceased.
The Statement of Claim form must name only one attorney. Statement of Claim forms listing more than one attorney, or a law firm, in the Attorney Information section will not be processed.
